Privacy Policy

Last updated: 4 November 2026

Evolve Neurodiversity Support Centre (“Evolve”, “we”, “us”) is operated by De Campos Family Pty Ltd (ABN 70 652 195 518). We provide behaviour support, developmental education and assessment services for children, young people and adults across the Clarence Valley and Coffs Coast.

We take your privacy seriously. This policy explains how we collect, use, store and share personal information, including health information. We follow the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and the Health Records and Information Privacy Act 2002 (NSW) and its Health Privacy Principles. When we provide NDIS supports, we also follow the NDIS Code of Conduct and other NDIS requirements that apply to us.

1. What information we collect

We only collect information we need to provide our services. Depending on how you deal with us, this may include:

  • Contact details: name, phone number, email address, postal address, and the details of parents, guardians, carers or other contacts.
  • Information about the person we support: name, date of birth or age, school or workplace, cultural background and communication needs.
  • Health and developmental information: diagnoses, developmental and medical history, behaviour, learning, assessment results, questionnaire answers, reports from other professionals, and notes we make while providing services.
  • NDIS and funding details: NDIS number, plan and funding information, and plan manager or support coordinator details.
  • Billing details: information needed to issue invoices and receive payment.
  • Website information: answers and contact details you submit through our online forms and screening questionnaires, and basic technical information collected by our website (see section 9).

Health information is “sensitive information” under privacy law. We only collect it with your consent, or where the law allows us to.

2. How we collect information

Where possible, we collect information directly from you. We may also collect it from:

  • a parent, guardian or carer, when we are supporting a child or a person who needs help to make decisions;
  • other people involved in the person’s care, such as GPs, paediatricians, psychologists, therapists, schools, plan managers and support coordinators, with your consent;
  • our website forms, email, phone, text and messaging services.

3. Online screening questionnaires and enquiry forms

Our website offers screening questionnaires (for example, for autism and ADHD) and enquiry forms (for example, about reading and learning concerns). When you submit one of these forms:

  • your answers, the questionnaire score and your contact details are sent by email to the Evolve team, and a copy is stored securely in our website’s form records;
  • we use this information to contact you, talk about your concerns and help plan any next steps, such as an assessment;
  • the information is treated as health information and handled under this policy;
  • we do not use it for marketing, and we do not sell it.

Screening questionnaires are not diagnostic tools. A result on our website is not a diagnosis and does not replace a full assessment by a qualified clinician.

You can ask us to delete a questionnaire or enquiry submission if you have not gone on to use our services. We will do so unless the law requires us to keep it.

4. How we use information

We use personal information to:

  • provide assessment, behaviour support, developmental education and related services;
  • communicate with you and the people you have asked us to work with;
  • write reports, plans and letters that you have requested or agreed to;
  • manage NDIS service agreements, claims, invoices and payments;
  • meet our legal, professional and NDIS obligations;
  • respond to enquiries, feedback and complaints, and improve the quality and safety of our services.

We only use information for another purpose if you consent, or if the law allows or requires it.

5. When we share information

We only share personal information with your consent, or where the law allows or requires it. This may include sharing with:

  • other health professionals, schools or services involved in the person’s care, when you have agreed;
  • the NDIA, plan managers and support coordinators, for NDIS-funded services;
  • the NDIS Quality and Safeguards Commission, where we must report certain incidents;
  • government authorities, where we are legally required to report, for example under NSW child protection laws, or where there is a serious threat to someone’s life, health or safety;
  • service providers who help us run our practice, such as email delivery, website hosting, cloud storage, practice management and accounting services. They may only use the information to provide their service to us.

We do not sell or rent personal information.

6. Information stored or processed overseas

Some of the service providers we use, such as email delivery services, website hosting and cloud software, may store or process information outside Australia, including in the European Union and the United States. We take reasonable steps to make sure these providers protect personal information in a way that is consistent with Australian privacy law.

7. How we store and protect information

We take reasonable steps to protect personal information from misuse, interference, loss, and unauthorised access, changes or disclosure. This includes password-protected systems, secure email and website services, restricted access to records, and locked storage for any paper documents.

Under NSW law we keep health information for at least 7 years after the last service we provide to an adult. If the information was collected when the person was under 18, we keep it at least until they turn 25. After that, we destroy or de-identify it securely.

8. Accessing and correcting your information

You can ask to see the information we hold about you or the person you care for, and ask us to correct it if it is wrong, out of date or incomplete. Please contact us in writing (see section 12). We may need to confirm your identity or your authority to act for someone else.

We will respond within a reasonable time, usually within 30 days. In some limited situations the law allows us to refuse access, for example where giving access would pose a serious threat to someone’s life, health or safety. If we refuse, we will tell you why in writing and explain how you can complain.

9. Our website and cookies

Our website uses cookies and similar technologies that are needed for it to work, stay secure and remember basic settings. Some features are provided by third parties, such as the Google reviews display, map links and the WhatsApp chat button. If you use these features, the third party’s own privacy policy also applies. For example, messages you send us through WhatsApp are handled under Meta’s privacy policy. You can set your browser to block or delete cookies, but some parts of the website may not work properly.

10. Anonymity

You can make a general enquiry without telling us who you are, or by using a different name. We cannot provide services, or follow up a screening questionnaire, without knowing who you are and how to contact you.

11. Data breaches

If a data breach is likely to cause serious harm, we will notify the people affected and the Office of the Australian Information Commissioner (OAIC), as required by the Notifiable Data Breaches scheme.
