We take reasonable steps to protect personal information from misuse, interference, loss, and unauthorised access, changes or disclosure. This includes password-protected systems, secure email and website services, restricted access to records, and locked storage for any paper documents.
Under NSW law we keep health information for at least 7 years after the last service we provide to an adult. If the information was collected when the person was under 18, we keep it at least until they turn 25. After that, we destroy or de-identify it securely.
You can ask to see the information we hold about you or the person you care for, and ask us to correct it if it is wrong, out of date or incomplete. Please contact us in writing (see section 12). We may need to confirm your identity or your authority to act for someone else.
We will respond within a reasonable time, usually within 30 days.
